Revised Case Mgt. Referral Protocol

The Council Case Management program serves clients who Vincentians identify and refer to case management staff for further assistance following a home visit. We have worked with individual Vincentians and Conference to streamline and make this referral process easy to understand and administer.

Revised New Referral Protocol:

Revised procedures for referral to Case Management team were presented at the April 26, 2014 Council meeting. In an effort to improve the efficiency of our intake process, we ask that all referrals to St. Vincent de Paul case management program be made by the President of the Conference.
