Case Management Locations

cmThe St. Vincent de Paul (SVdP) Case Management Program assists people transition from barriers created by poverty, incarceration, homelessness and other distressed situations into positive and hope-filled new beginnings. The goals are to help people regain self-sufficiency, cultivate positive relationships, and begin to lead responsible lives in their communities. The most serious problems are mental illness, unemployment, histories of addiction or incarceration, and lack of affordable housing.

SVdP Case Managers work with individuals weekly, one-on-one, to help them gain housing, employment or education, to meet their Individualized Action Plan goals. Our Case Managers utilize Cognitive Behavioral Therapy and Motivational Interviewing, evidence-based approaches to case management that demonstrate proven results in generating positive participant outcomes. These therapies focus on changing an individual’s negative thoughts into more positive thinking, creating constructive behavioral patterns and outcomes with the goal of self-sufficiency.
