Summary: Provide specialized and progressive case management services helping people with disabilities find and/or maintain housing. This position’s duties involve housing, other behavioral needs and some legal advocacy with landlords. This position is part of the Amerigroup/Medicaid Supported Housing Benefit– Individual Placement and Support (IPS) program.
Essential Duties and Responsibilities include the following; however, other duties may be assigned:
- Provide pre-tenancy support, including but not limited to, creating a person-centered goal plan, making referrals and connections to social services to help with finding and applying to housing; identifying short and long-term goals.
- Provide linkages and referrals to primary care, health homes, substance treatment, mental health providers, vision, dental, nutritional, vocational, education, employment, and volunteer support etc.
- Assistance with navigating and monitoring application process with agencies.
- Provide support in independent living skills (i.e. financial literacy).
- Use motivational interviewing to provide outreach to potential clients and explore if program enrollment could assist them in reaching their housing stability goals.
- Provide training and resource support related to landlord/tenant laws and lease compliance (including ongoing support with household management).
- Provide benefits education, counseling, and planning.
- Maintain physical case files and ensure database entry and outcomes in accordance with agency standards.
- Participate in team meetings, training, and other professional development needed to ensure program quality.
- Work collaboratively with other staff to identify resources and services that can be used to better serve clients.
- Provide individualized case management for clients as referred by other agencies and conferences.
- Employ crisis management skills where necessary to assist clients in difficult situations.
- Manage a caseload of 20-25 households in varying stages of the program curriculum.

Bachelor’s degree or higher in Human Services or related field from an accredited college or university and at least 1 year of experience providing housing services and support.
Language Ability/Communication Skills
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization. Must have strong verbal and written communication skills. Bilingual in English and Spanish preferred.
Ability to use compassionate, reflective listening strategies to engage families of low-income backgrounds.
- Knowledge of case management practices.
- Knowledge of King County labor market, vocational training programs, WorkSource system, and other local resources.
- Must have strong knowledge and experience in maintaining confidentiality.
- Knowledge of community resources and confidence to handle complicated situations.
- Ability to make sound judgment without direct supervision.
- Willingness to work as part of a team and independently.
Ability to travel
Candidate must have access to a vehicle to be able to travel independently between multiple worksites within a day.
Ability to accurately record addresses, and to use Google Maps and printed wall map to locate any address in King County. Ability to detect and correct misspellings, wrong street types (e.g.: Ave. vs St.), wrong directional indicators (SW SE N etc.), and incorrect zip codes to ensure that the correct client location is identified. Understanding of points of the compass and ability to use NE SE SW etc. to rapidly narrow down a map search.
Ability to do basic arithmetic (add, subtract, multiply and divide), have a sense of scale (e.g.: to know that 12000s block is 30 blocks north of the 9000s block), and know odd and even numbers. Ability to handle numbers accurately without transposing digits.
Ability to handle incomplete or unreliable information (e.g.: addresses, phone numbers) and ask appropriate questions or look up maps or other references to get accurate information. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Proficiency with Microsoft Excel and Client-based data entry, Outlook email, Google Maps and internet searches required. Proficiency with Microsoft Word is an advantage.

This job has no supervisory responsibilities of staff—some supervision of volunteers required.
